WHAT DO THESE CHOICES MEAN
Discount Card
Discount cards are offered as a way to lower the cost of dental care. These cards are especially attractive for people who do not have dental insurance for these services or who have inadequate dental insurance. Discount cards allow for unlimited use of services by using a dental network and are based on a discounted fee schedule. The cost of your membership card is small compared to the potential savings for you and your family.
Dental HMO
Dental Health Maintenance Organization. An organized system of dental health care that provides comprehensive and affordable care to you through a network of highly qualified dentists. There are no waiting periods, calendar year maximums or deductibles and no claim forms. With these plans, you are responsible for the copayment for each service.
Indemnity
An indemnity plan is commonly known as a traditional plan. If you select an Indemnity plan you have the freedom to visit any dentist. Indemnity plans typically pay a percentage of "usual,customary and reasonable" charges for covered services. Many dental indemnity plans also require a deductibe and waiting periods before covering certain services.
Premier (Indemnity)
A Premier (Indemnity) plan is commonly known as a traditional plan. If you select a Premier plan you have the freedom to visit any dentist. Premier plans typically pay a percentage of "usual,customary and reasonable" charges for covered services. Many dental premier plans also require a deductibe and waiting periods before covering certain services.
PPO-Fee Schedule
With a PPO, you will receive dental care from a dental network. The participating dentist agrees to charge you less than the usual fees therefore providing you with more savings. You may choose to see a dentist who is not part of the dental network, but reimbursements will be limited to a fee schedule with any balance due being the responsibility of the member. PPO's do include a calendar year maximum, a deductible and have waiting periods on certain procedures.

PPO

A PPO plan provides the opportunity to reduce your out-of-pocket costs by using an in-network provider, yet you are always free to select a dentist not associated with the PPO. If you choose to see a dentist who is not part of the dental network, your claim allowance is based on the Usual, Customary, and Reasonable charges for 9 out of 10 dentists in the area where dental services were performed. PPO's do include a calendar year maximum, a deductible and have waiting periods on certain procedures.

Scheduled

A scheduled plan or scheduled reimbursement plan reimburses you for a specific amount based on a fee schedule with any balance due being your responsibility. Scheduled plans do include a calendar year maximum, a deductible and have waiting periods on certain procedures. A Scheduled plan shows every covered procedure and the amount you will be reimbursed for each procedure. For example, if the insurance reimburses $47 for a cleaning, and your dentist charges you $50 for a cleaning, you would be responsible for the difference of $3.
