APPLICATION PROCESS

APPLICATION TIME FRAME

- Please note: If you currently have a BCBS plan and don't make a plan change, your plan should roll over into 2017. This plan will have a new
  premium and out-of-pocket benefit amounts. Refer to the plan information BCBS mailed you concerning this. Continue with the existing plan (if
  offered) 
or run a new BCBS quote to compare other options to meets your needs for 2017.
 
- BCBS HMO Off Marketplace applications:

  After submitting the application, it’s assigned an initial application ID#, generally within 2-4 days.
- BCBS HMO On Marketplace applications:
  After submitting the application at Healthcare.gov and received at BCBS, it’s assigned an initial application ID#. Process generally takes 5-7 days.
- Molina Healthcare HMO On Marketplace applications:
  After submitting the application at Healthcare.gov and received at Molina, it’s assigned an initial application ID#. Process generally takes 5-7 days.

- Scott and White HMO or PPO Off Marketplace applications:
  After submitting the application, it’s assigned an initial application ID#, generally within 2-4 days.

TO FIND INITIAL APPLICATION ID#
If you didn’t receive an email with this information, contact Member Service after the allotted time.

PAY INITIAL PREMIUM – TO COMPLETE ENROLLMENT
- BCBS On Marketplace applications:
  After confirming your application is at the insurance company, you can make the first premium payment to activate your policy ID# at that time.
- BCBS Off-exchange applications:
  You pay the first month’s premium during the application process.
- Molina Healthcare On Marketplace applications:
  After confirming your application is at the insurance company, you can make the first premium payment to activate your policy ID# at that time. 

- Scott and White HMO or PPO Off Marketplace applications:
  After confirming your application is at the insurance company, you can make the first premium payment to activate your policy ID# at that time.

MEMBER LOG IN
After your policy is activated and you have a policy ID#, you can register/log in to the member portal.


PRIMARY CARE PHYSICIAN (PCP)

• BCBS HMO On Marketplace applications:
  If you didn’t select a PCP via the enrollment, once you have a policy ID#, call Member Service or log in to the member portal to add your PCP.
• BCBS HMO Off Marketplace applications:
  You can add your PCP during the application process.
• BCBS HMO On & Off Marketplace:
  - You need to select a PCP for each member on your policy. Visit the provider network to view an online list of PCPs. Clink on the physician’s name
    (link) to find the PCP 10-digit number.

  - If you didn’t select a PCP when you applied, call Member Service or log in to your member portal to add your PCP.
  - If you don’t select a PCP, you will be assigned one.
  - If you want to change the PCP during the year, call Member Service or log in to your member portal to make this change and verify when new
    PCP is active. Generally, the new PCP should be available the following month the change was made.

• Molina Healthcare HMO On Marketplace applications:
  You do not need to select a PCP.
• Scott and White HMO or PPO Off Marketplace applications:

  You do not need to select a PCP.


PROVIDER NETWORK
Visit the insurance company website to find In-network Medical providers and Pharmacies.

PRESCRIPTION DRUG LIST
Find your medication and assigned tier (generic to specialty). Review your plan Summary of Benefits to determine the payment level.

ID CARD AND POLICY
- Members receive plan materials only after paying the first month’s premium. Please allow 7-10 days to receive this information.
- To print the temporary ID card / check your plan details / view claims and more, log in to the member portal.
